Breastfeeding Your Newborn (0-6 weeks)

Breastfeeding Your Newborn (0-6 weeks)

The first six weeks of your newborns life are the most important, and also the most difficult. If you can make it through these first six weeks you are doing great! Setting small goals (3 months might be a next goal) to help keep you motivated and whetherd is always a good idea. Nursing on demand is the way to go straight from the beginning. Being educated as much as you can before your baby is born is a great idea. Hopefully these links will help you wether your baby is still in the womb, or baby is already here!

When To Pump
  • if you will be returning to work, start about a week before you are to return and pump once a day and store that milk in the freezer. When you return to work, the milk you pump that first day will be used for the following day's bottles so there isn't much need for a huge freezer stash. Also, make sure your baby's care givers know about paced bottle feeding (see our file). Babies drink 1-1 1/2 oz per hour since last feeding so that would typically be a 3-5oz bottle every 3 hours. Breastmilk changes to meet babies needs so there is no need to increase the amount given at a bottle feeding.
  • if you are engorged you can pump or hand express JUST ENOUGH to get relief. Completely emptying your breasts will cause them to continue to make that amount and you will end up with oversupply and have to deal with those issues.

Infant Sleep For The Breastfed Baby

Infant Sleep for the Breastfed Baby

All babies wake up several times a night to nurse during the early weeks and this is COMPLETELY NORMAL!!! It is NOT NORMAL for your newborn to sleep more than two hours between feedings at 2 weeks old or younger. If your baby is younger than 2 weeks and sleeping longer than 2 hours YOU NEED TO BE WAKING HIM/HER to nurse!!! After that two week period you can relax a bit and start fully NURSING ON DEMAND. Bedsharing or Co-sleeping is often recommended for breastfeeding moms because this allows everyone to get more rest/sleep. Sleeping through the night is not really the best thing for your baby! And did you know that sleeping through the night is considered the baby sleeping 5-8 hours uninterrupted. NIGHT WEANING should NOT be done before a year old as breastmilk is the best form of nutrition up to, and beyond that age. Babies should not be on schedules as that can damage moms suppy and cause baby to not get enough milk, especially while exclusively breastfeeding. Sleep training methods such as the Ferber method of CIO are very dangerous for infants and there is a lot of research to support *not* using these sleep training methods (see links below).

Why do babies wake at night?
Babies wake at night for many reasons, and they often start waking at night after sleeping through for a few weeks or months. Some of the reasons for night waking (in no particular order) are:
  • baby wants more time with mom
  • teething
  • developmental advances (for example: waking more often right before or after learning to turn over, crawl or talk)
  • illness, allergy, diaper rash, eczema
  • hunger (including growth spurts)
  • reverse cycling: Some babies whose moms are away during the day prefer to reject most/all supplements while mom is away, and nurse often during the evening and night. If mom is very busy during the day or if baby is very distracted, this can also lead to reverse cycling.
  • Growth spurts and sleep pattern changes can also cause baby to wake more at night

Studies of Normal Infant Sleep

Waking A Newborn To Nurse
If your baby is younger than 2 weeks, you should be waking him/her to nurse EVERY TWO HOURS.

Nursing on Demand
Recommended for breastfed babies.

Co-Sleeping or Bedsharing
This is recommended for breastfeeding moms when it's able to be done safely so that everyone can get more rest/sleep.

"Sleeping Through The Night"
Breastmilk digests in less than 2 hours, so don't be surprised when your excluisvely breastfed child is waking frequently through the night to nurse!

Sleep Regression
Many moms find the Wonder Weeks app to be very helpful in predicting when their babies might go through a sleep pattern change. Solids, even rice cereal is NO LONGER RECOMMENDED BEFORE 6 MONTHS of age by the AAP and many other sources, see the Starting Solids file linked just below.

Will Solids or Formula Help My Baby Sleep?
In short, there is no proof that these things will help baby sleep better. Formula can actually cause baby to be awake MORE since they are not used to digesting it on a regular basis.

Why You Shouldn't Schedule Sleep & Feedings
As already discussed on this file, nursing on demand is ideal for breastfeeding moms and babies so that babies always get enough milk and so that mom's always have good supply.

Why you should avoid CIO (Cry It Out) & other 'Sleep Training' Methods
Crying is your babies ONLY form of communication before 1 year, sometimes even longer. Why ignore your child's only form of communication? There is a LOT of evidence that supports *not* using the CIO method in particular.

Prenatal Yoga
Squats. Stop doing squats at 36 Weeks though, to allow baby to get and stay in a good position for labor, then start again once you are in active labor to help baby move down: http://birthbootcamp.com/pregnancy-exercises-for-a-great-birth/

Consider having a close friend of family member throw you a Birth Blessing Ceremony: http://codenamemama.com/2011/10/26/mother-blessings/
Surround yourself with affirmation, support, and encouragement. Do what it takes to get the birth you desire.

Getting Labor Started/More Productive Contractions:
***Remember: It's best to wait until baby is ready to come and to work with your body and not force labor prematurely. Due Dates are an estimate only. It is recommended to not attempt any means of inducing labor until 40 or 41 Weeks, natural methods included.
Prenatal Massage
Rest and Nap as much as possible. Being well rested and relaxed is important for letting labor begin.
Have a make out session with your significant other and/or have sex (as long as your water hasn't broken yet).
Go for long walks and stay active.
Walk up and down stairs. To get your body doing deep lunges, take them 2 at a time for 15 minutes at a time, then rest and repeat. This might help move baby further down.Chiropractic Care (only a Chiropractor who is skilled at working with Pregnant women).
Acupuncture
Pray/Meditate
Do the exercises on spinningbabies.com website. If Baby is not positioned properly for birth, it can hold things up. So work on getting Baby into an optimal position so labor can begin.
Nipple stimulation with a Breast Pump: 2 min on, 3 min off, and repeat for 2 hours to get steady Oxytocin flowing.
Make Parsley Tea. Steep fresh Parsley in hot water for a few minutes, strain, then drink.
Apply Clary Sage & Myrrh Essential Oils (diluted with a carrier oil) to the inside of your ankles and abdomen for labor/contraction support. Please educate yourself on the proper use of Essential Oils. They are potent and can be dangerous when used incorrectly.
Last resort: Take Castor Oil, but it can really “clean you out” and make delivery messy for some women.
Last resort: Have Midwife or Dr strip membranes, and then go for a walk right afterwards.

Support & Pain Management During Labor:
Hire a Doula! A Birth Advocate can make all the difference in achieving the birth you want. Many regret never hiring one, but it's rarely heard that anyone actually regretted having one there.
Only have people in the delivery room who are supportive of Natural Birth, INCLUDING your OB or Midwife. If you desire a Natural Birth and they are not supportive, then switch to a different practitioner, no matter how late in the game you are. It will be worth it.
Embrace each contraction, knowing what they are trying to accomplish, so they are more productive.
Use a Birthing Ball.
Get in Water, i.e. the shower, pool, or tub, or consider a waterbirth.
Squat. Squatting uses gravity to help Baby move down, intensifies the urge to push, and can increase Pelvic Outlet by up to 15%.
Use a Rebozo during Labor.
Acupressure: http://www.mamanatural.com/how-to-do-acupressure-when-youre-in-labor
Try these different Positions: http://www.transitiontoparenthood.com//ttp/parented/pain/positions.htm
Massage/Counterpressure
Breathe Deeply
Use Heat. DIY Heating Pad: http://www.budgetsavvydiva.com/2012/12/diy-heating-pack-sew/
Turn off the Lights: Melatonin synergizes with Oxytocin to enhance labor contractions.
Eat a Honey stick/spoonful of honey for a boost of energy if needed during labor.
Eat
Stay Hydrated
Drink Coconut Water to restore Electrolytes naturally. Or DIY  labor-aid: http://www.mommypotamus.com/how-to-make-a-labor-aid-electrolyte-drink/
Homeopathic Carbo Veg is great for nausea during labor

Essential Oils for Labor & Postpartum:
***Please educate yourself on the proper use of Essential Oils. They are potent and can be dangerous when used incorrectly. Always dilute with a Carrier Oil.

Single Oils: Black Pepper, Clary Sage, Frankincense, Geranium, Ginger, Lavender, Lemon, Myrrh, Peppermint, White Fir.

Types of Blends: Calming Blend, Digestive Blend, Grounding Blend, Invigorating Blend, & Pain Relief Blend.
Peppermint, Ginger, or a Digestive Blend to support a nauseated Mother during labor. Usually just inhaling over the open bottle or putting a drop on a tissue to sniff is adequate.
Clary Sage & Myrrh rubbed on abdomen and inside of ankles will bring on stronger and more consistent contractions when labor has stalled.
2 drops of Black Pepper or a Pain Relief Blend on lower back for supporting back labor.
A Grounding Blend and/or a Calming Blend diffused during labor for a peaceful and calming effect.
A Grounding Blend on bottoms of feet through labor, as well as daily at rise and bedtime during Postpartum.
Serenity for calming and relaxation during labor, it's great rubbed on back of neck and wrists. Use anytime during postpartum too.
Any citrus Essential Oil Single or Invigorating Blend diffused or on wrists can support a tired laboring Mama.
Lavender and White Fir rubbed on the abdomen to reduce afterbirth pains.

Managing Afterbirth Pains:
White Fir & Lavender Essential Oils applied to abdomen for pain support. Please educate yourself on the proper use of Essential Oils. They are potent and can be dangerous when used incorrectly. Always dilute with a Carrier Oil.
The Red Raspberry Leaf Tea (mentioned above) helps prepare your body ahead of time, so that your afterbirth pains will be easier to manage.

  • Do the exercises on spinningbabies.com website. If Baby is not positioned properly for birth, it can hold things up. So work on getting Baby into an optimal position so labor can begin.
  • Nipple stimulation with a Breast Pump: 2 min on, 3 min off, and repeat for 2 hours to get steady Oxytocin flowing.
  • Make Parsley Tea. Steep fresh Parsley in hot water for a few minutes, strain, then drink.
  • Apply Clary Sage & Myrrh Essential Oils to the inside of your ankles and abdomen for labor/contraction support. Please educate yourself on the proper use of Essential Oils. They are potent and can be dangerous when used incorrectly. Always dilute with a Carrier Oil.
  • Last resort: Take Castor Oil, but it can really “clean you out” and make delivery messy for some women.
  • Last resort: Have Midwife or Dr strip membranes, and then go for a walk right afterwards.

Support & Pain Management During Labor:

  • Hire a Doula! A Birth Advocate can make all the difference in achieving the birth you want. Many regret never hiring one, but it's rarely heard that anyone actually regretted having one there.
  • Only have people in the delivery room who are supportive of Natural Birth, INCLUDING your OB or Midwife. If you desire a Natural Birth and they are not supportive, then switch to a different Practitioner, no matter how late in the game you are. It will be worth it.
  • Embrace each Contraction, knowing what they are trying to accomplish, so they are more productive.
  • Use a Birthing Ball.
  • Get in Water, i.e. the shower, or tub, or consider a waterbirth.
  • Squat. Squatting uses gravity to help Baby move down, intensifies the urge to push, and can increase Pelvic Outlet by up to 15%.
  • Use a Rebozo during Labor.
  • Acupressure: http://www.mamanatural.com/how-to-do-acupressure-when-youre-in-labor
  • Try these different Positions:http://www.transitiontoparenthood.com//ttp/parented/pain/positions.htm
  • Massage/Counterpressure
  • Breathe Deeply
  • Use Heat. DIY Heating Pad: http://www.budgetsavvydiva.com/2012/12/diy-heating-pack-sew/
  • Turn off the Lights: Melatonin synergizes with Oxytocin to enhance labor contractions.
  • Eat a Honey stick/spoonful of honey for a boost of energy if needed during labor.
  • Eat
  • Stay Hydrated
  • Drink Coconut Water to restore Electrolytes naturally. Or DIY  labor-aid:http://www.mommypotamus.com/how-to-make-a-labor-aid-electrolyte-drink/

  • Peppermint, Ginger, or a Digestive Blend to support a nauseated Mother during labor. Usually just inhaling over the open bottle or putting a drop on a tissue to sniff is adequate.
  • Clary Sage & Myrrh rubbed on abdomen and inside of ankles will bring on stronger and more consistent contractions when labor has stalled.
  • 2 drops of Black Pepper or a Pain Relief Blend on lower back for supporting Back Labor.
  • A Grounding Blend and/or a Calming Blend diffused during labor for a peaceful and calming effect.
  • A Grounding Blend on bottoms of feet through labor, as well as daily at rise and bedtime during Postpartum.
  • Serenity for calming and relaxation during labor, it's great rubbed on back of neck and wrists. Use anytime during postpartum too.
  • Any citrus Essential Oil Single or Invigorating Blend diffused or on wrists can support a tired laboring Mama.
  • Lavender and White Fir rubbed on the abdomen to support Afterbirth pains.

Managing Afterbirth Pains:

  • White Fir & Lavender Essential Oils applied to abdomen for pain support. Please educate yourself on the proper use of Essential Oils. They are potent and can be dangerous when used incorrectly. Always dilute with a Carrier Oil.

Royal Australasian College of Physicians http://www.racp.edu.au/index.cfm...  "After reviewing the currently available evidence, the RACP believes that the frequency of diseases modifiable by circumcision, the level of protection offered by circumcision and the complication rates of circumcision do not warrant routine infant circumcision in Australia and New Zealand." (almost all the men responsible for this statement will be circumcised themselves, as the male circumcision rate in Australia in 1950 was about 90%. "Routine" circumcision is now *banned* in public hospitals in Australia.)

British Medical Association http://bma.org.uk/.../Practical.../Ethics/Circumcision.pdf "to circumcise for therapeutic reasons where medical research has shown other techniques to be at least as effective and less invasive would be unethical and inappropriate."

The Royal Dutch Medical Association http://knmg.artsennet.nl/.../Nontherapeutic-circumcision... "The official viewpoint of KNMG and other related medical/scientific organisations is that non-therapeutic circumcision of male minors is a violation of children's rights to autonomy and physical integrity."

German Pediatric Association http://www.intactamerica.org/german_pediatrics_statement(very long, but very much against circumcision, and includes the following) "Therefore it is not understandable that circumcision of boys should be allowed but that of girls prohibited worldwide. Male circumcision is basically comparable with FGM types Ia and Ib that the Schafi Islamic school of law supports"

http://pediatrics.aappublications.org/.../peds.2012-2896... "The other claimed health benefits, including protection against HIV/AIDS, genital herpes, genital warts, and penile cancer, are questionable, weak, and likely to have little public health relevance in a Western context, and they do not represent compelling reasons for surgery before boys are old enough to decide for themselves." (signed by 38 senior physicians, about half of them presidents or chairs of national paediatric or urological organisations)
